Prescription Exemption

If you have a long-term condition, you may be eligible for free NHS prescriptions.

You can check if you are eligible here.

If you are entitled to free prescriptions, please collect an FP92A (Prescription Exemption Application Form) from reception, fill it in and then return it to reception. This form will then be completed by a GP or medical representative who has access to your medical records and they will send the form off on your behalf.

You will then receive your exemption certificate in the post – so please make sure that your contact information is correct and up to date.

If you are pregnant you may also be eligible for free NHS prescriptions. You can collect a FW9 (Maternity Prescription Exemption Application Form) from the surgery and follow the same steps as above.
