How to Request a Sick Note

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill, but they can ask you for a self-certification sick note. You can collect a self-certification form from the surgery. You should fill this in and hand it to your employer.

Alternatively, you can click here to fill out the form online on the Government website.

If you’re off work sick for more than 7 days, your employer will usually ask for a sick note (or Statement of Fitness for Work) from a healthcare professional. If you require a sick note, please contact the surgery at 8.00am to make an appointment with the GP.

To renew an existing sick note please call the surgery after 10.30am. Please note that your GP may ask for you to attend a review.
